Use this form to report a suspected or confirmed cyber incident at your school or trust.

We will use this information to understand the types of cyber incidents affecting schools and their impacts.

We will not share your information outside the Department for Education.​

Before you start

You cannot save and return to it later, so have all your information ready.

You will be asked for your:

• email address
• information about the incident
• Unique Reference Number (URN) – if you are a school

Find your school's URN on the Get Information About Schools Service.

To get a copy of your answers by email, you’ll need a GOV.UK OneLogin. If you do not have one, we'll give you the option to create one at the end of this form.
